Welcome to the CDH Foundation

My name is Rebecca Schroeder and I am the Executive Director of the the CDH Foundation. As a CDH mom I am intimately familiar with the ups and downs of this diagnosis. No matter where you are in your CDH journey, we strive to serve you where you are and find or provide the resources you need when you need them.

On the following page is our grant application, once completed we will need you to provide us with a letter from your hospital confirming a CDH diagnosis, reason for re-hospitalization and projected length of stay. The letter should be signed, dated and list the contact information of the individual signing. Please email those documents to becca@thecdhfoundation.org.
