Loss Grant Application

My name is Rebecca Schroeder and I am the Executive Director of the the CDH Foundation. I am so very sorry for the loss of your sweet baby. Thank you for trusting us to help your family through this difficult time. We want to be a shoulder to both lean and cry on as you navigate this journey.

In addition to grants we also provide a peer led loss support group called Grief Share. We meet monthly on zoom and we also have a very active chat for families to connect. If you would like more information visit thecdhfoundation.org/griefshare

On the following page is our grant application, once completed we will need you to provide us with either the death certificate or a letter from your hospital. The letter should include your name, your child’s name, date of birth, date of death, CDH diagnosis and should be signed, dated and include the contact information of the individual signing.
