My name is Rebecca Schroeder and I am the Executive Director of the the CDH Foundation. As a CDH mom I am intimately familiar with the ups and downs of this diagnosis. No matter where you are in your CDH journey, we want to find or provide the resources you need when you need them.

On the following page is our transfer grant application, once completed we will need you to provide us with a letter from the hospital confirming the CDH diagnosis, the name of the hospital you are transferring to and the reason for transfer. The letter should be signed, dated and list the contact information of the individual signing. Please email those documents to becca@thecdhfoundation.org.
